Home healthcare ensures that patients can continue to receive high-quality medical care in the comfort of their own home after surgery, illness, injury, medical procedures, or in cases of chronic conditions.
Who is eligible for home healthcare?
- Patients with chronic illnesses and mobility limitations that prevent them from visiting a medical facility.
- Patients discharged from a hospital or day hospital following surgical procedures.
⚠️ Please note: patients must obtain the medications prescribed by their family doctor or specialist that are necessary to provide home healthcare services and medical procedures at home.
⚠️ A referral from a family doctor or a hospital specialist is required.
